• Picture of the front and back of a valid updated US drivers license.  

• Picture of a valid updated vehicle registration.

• Picture of valid updated vehicle insurance cards in amounts and of types required by law to provide the Contracted Services and cover Contractor during performance of the Contracted Services, at their own expense. 

Contractor MUST maintain updated information the entire time working as an independent contractor.
